At the moment, it is not possible to add the 'File Upload' button to forms; only a button with 'Add Attachment' appears at the end of the forms. The introduction of this button would be quite useful, even for multiple additions, because in this way, rules could be implemented to check whether an attachment exists or not.

This allows attachments of any type to be provided in exact place in the form (where you put the Survey control).
I remove all the voting options but one, and change the text to "->"
