Description Field for Form

I feel that we need a description field for a form. Right now we only have form Name. With form description field, we able to give more information / remarks - like what is the purpose for that form and other notes.

indicate the escalation info in the pdf report export if one form is escalated to another form

Move QR Code after placement

We have to be able to move the QR Code after it is placed. Right now, the entire sheet has to be deleted and reimported back in to change the location of a QR code by placing a completely new one.
